A 54-year-old male presented with severe aortic regurgitation. Physical examination showed severely infected lower leg ulcers(LLU). Giant aortic aneurysm( 110 mm) with Stanford type A aortic dissection was revealed by computed tomography. We treated his infected legs first and on the 22nd day he underwent modified Bentall operation. Postoperative course was uneventful and he moved to other hospital for skin grafting of the lower extremities. Pathological study showed no specific change in the aorta.Coronary aneurysms are relatively rare
