A mixture of abdominal malrotation and distal cholangiocarcinoma is known as a rare problem and poses some troubles in medical management. We present an incident of a patient with asymptomatic nonrotation for the midgut with a concomitant distal cholangiocarcinoma who underwent effective pancreaticoduodenectomy. A 52-year-old Sudanese man delivered to our hospital with modern painless jaundice involving dark urine, pale stool, and itching for the past
