We report a rare case of dysphagia after occipitocervical fixation, and aim to emphasize the importance of occiput-C2 angle. The kyphotic change of occiput-C2 alignment after operation might lead to dysphagia due to decreased oropharyngeal cross-section area and increased basal pressure of upper esophageal sphincter.
